How to Schedule Your EOPS Counseling Appointment

To schedule your EOPS counseling appointments, complete the Request for EOPS Counseling Form for your assigned EOPS Counselor.  All students are assigned to EOPS Counselors by last name.

Your appointments will be scheduled by EOPS Technicians based on the information you provide on the Request for EOPS Counseling Form below.

After you have completed and submitted a request form for your assigned EOPS Counselor, your EOPS Technician will schedule an appointment based on the availability you provided (in person, by phone, or online, via Cranium Cafe). The option you choose for your first appointment will be the one used for ALL appointments in the semester.

You will receive an automatic appointment confirmation via email/text. It is important that you check your AVC email often for communication from EOPS.

Note:  All EOPS students are required to complete a total of three EOPS counseling appointments each semester.

Missed Appointments

If you miss a scheduled appointment twice in one contact period, you will not be able to reschedule for a third time, and that appointment will then be considered a missed appointment. This is important to note, as missing appointments can result in the loss of grants and other program opportunities, and may lead to a suspension or termination from the program.
